I think it has something to do with the fact that demoman hands aren't team colored, but the medic wears team colored gloves. So the demoman would need to have the same skintone on both teams, and the Nazis have no blacks.
How come the Medic can be black but the Demoman can't?

Also the Demoman is Scottish. Why would he act like a ghetto? After reading your post I have all doubt you have simple knowledge of this game if you actually thought the Scout was Italian. And Italian stereotypes don't carry flags. They make Pizza and sound like Mario.
